**Before completing this application, please take note of the following**

1. All Applications should have:

A) A definite call, confirmed to others by the witness of the Holy Spirit.

B) An understanding of, and faith in, the purpose of God for this present hour as generally accepted by Elim Fellowship Canada. View Constitution and By-Laws Here

C) A desire and dedication to give themselves wholly to the ministry.

2. All parts of this application that are applicable must be completed. This includes a non-refundable $50 application fee to process this registration.

3. If more room is needed then this form allows, please send an email after you’ve submitted this application with the additional information.

4. It is the applicants responsilibity to have their references fill out the appropriate form.
